Equality Health supports the work of primary care providers and enables a transition to value-based care, without the need for practices to build infrastructure from scratch. We secure value-based contracts with sponsoring payers, then bring those opportunities to local providers.

This opens up value-based payment models, particularly among diverse and often underserved populations, such as those covered by Medicaid. We then dig into the hard work with practices to close care gaps, optimize performance, and improve quality.

We help participating providers opt into and then optimize value-based care contracts.  We help smaller, independent practices adopt the technology they need, without the need of investing in vast infrastructure of the largest medical groups and systems. This helps us support PCPs in improving patient outcomes, regardless of which plan the patient is enrolled in.

We ask that our providers participate in the exchange of health information, complete cultural care training, and engage in our care coordination platform, CareEmpower®, to close care gaps.

There is no cost to joining our network.

Only if you want. Providers in our network have the choice to opt-in or opt-out of any new health plan agreement, without impacting existing nonduplicative network or health plan agreements.
