Equality Health and UnitedHealthcare launched a new program to improve the quality of health care for people enrolled in UnitedHealthcare’s Medicaid plans in the Phoenix area by promoting a team-based, holistic approach to patient care.
Through this program, Equality Health’s integrated network of physical, behavioral and community-based care providers will deliver quality, culturally effective and coordinated health care to their patients. The collaboration improves the ability to identify social and cultural factors that put patients at high risk and help them reduce avoidable emergency room visits and readmissions to the hospital, manage chronic health conditions and take their needed medications.
Equality Health has developed its innovative approach to care management using technology, services and network designs to organize a better health care delivery system for diverse communities that have struggled with integrating into the traditional one-size-fits-all system.
