Louisiana Healthcare Connections Joins Texas-Based Superior HealthPlan to Become the Latest Centene Company to Address the Complex Care Needs of Diverse, Underserved Communities with Equality Health
PHOENIX, Az., BATON ROUGE, La. and AUSTIN, Tx. (Sept 10, 2024) – Equality Health, a national leader in value-based care with its Medicaid-first model, and Louisiana Healthcare Connections, a managed care organization serving members across Louisiana and a company of Centene Corporation (“Centene”) [NYSE: CNC], today announced plans to support providers in advancing value-based care (VBC) in the state. The collaboration is aimed at closing health equity gaps for Medicaid members and is an expansion of the relationship Equality Health has developed with Superior HealthPlan (“Superior”) to support providers in Texas, beginning in 2021.
Equality Health, Louisiana Healthcare Connections and Superior have aligned missions to address health inequities among diverse and historically marginalized populations at a local level.
“We look forward to working with Equality Health to continue to drive strong health outcomes for our members,” said Jamie Schlottman, plan president and CEO of Louisiana Healthcare Connections. “Equality Health’s approach aligns well with Louisiana Healthcare Connections’ initiatives to eliminate health disparities and create a more inclusive healthcare system.”
Equality Health is helping transform the current state of healthcare by aligning, engaging, and empowering providers and health plans to focus their work on the members and communities that need it most. For example, in Texas, Superior and Equality Health have collaborated to improve the level of quality care accessible to Medicaid members. This includes a 5% increase in follow-up care for children prescribed attention-deficit/hyperactivity disorder medication as well as 4% increases in cervical cancer screenings, immunizations for adolescents, and blood pressure control for diabetic patients.
Since 2022, Equality Health has worked closely with Superior to expand access to critical healthcare services in Uvalde, Texas, a community impacted by a school shooting. This includes a partnership with the Equality Health Foundation to help develop Uvalde’s new health and wellness center.
“We are proud to see the positive impact that our collaboration with Equality Health has had in communities across Texas and are excited to see this important work extend into our neighboring state of Louisiana,” said Mark Sanders, CEO of Superior HealthPlan and regional senior vice president of Centene’s markets in the South.
By fusing technology, coaching, clinical expertise, financial support and a deep understanding of VBC, Equality Health helps improve whole-person care. According to a recent report on the VBC enabler landscape, “Equality Health’s focus on Medicaid beneficiaries gives a unique shape to its care model…
successful population health management within Medicaid depends heavily on risk stratification, prioritizing interventions, and matching those interventions to the level of need.”
“We are thrilled to extend our joint successes in Texas, and team up to assist primary care providers and Medicaid beneficiaries in Louisiana,” said Benjy Green, regional market president of Equality Health. “Together, we’ll continue to address healthcare disparities, improve outcomes and drive meaningful change.”
