We’re advancing health equity for Medicare beneficiaries in Arizona. PCPs and specialists can partner with our Accountable Care Organizations participating in CMS’ Realizing Equity, Access, and Community Health (REACH) Model or ACO MSSP program to improve patient outcomes and empower their practice with local support and opportunity for additional revenue.
